Carers - What is a care plan?

After your social worker has carried out your assessment and any services you need have been agreed, your social worker will then draw up a Care Plan with you. The Care Plan will detail:   

  • the services that you will get, and which organisation or individual will provide them; 
  • how often you will get them, and for how long; 
  • which members of staff will be responsible for making sure you get these services; and 
  • when the services will be reviewed, to make sure your needs continue to be met.                    

You will be given a written copy of your Care Plan in a folder which will also include a copy of your assessment of need.   

If you have agreed with us that you want to receive Direct Payments, you will be asked to produce your own Care Plan. This will then include details of the services you will arrange with your Direct Payments.   

If you asked your local Carers Association for help, they will discuss your needs with our social workers. A social worker might then visit you to complete an assessment using the information your Carers Association has already put together with you.
