After your worker has carried out your assessment, they will then agree with you how best to meet your needs. We cannot provide help to everyone so we use assessments to tell us who needs services the most. We will use the needs identified in your assessment alongside guidelines called 'eligibility criteria' to decide if you are eligible for help from us. Once any services you need have been agreed with you, your worker will draw up a care plan with you. If you have had a joint assessment, this care plan will be a joint one for you and the person you look after. If you have had a separate assessment, you will have your own care plan.
The care plan will tell you:
- the services that you will get, and which organisation or individual will provide them;
- how often you will get services, and for how long;
- which members of staff will be responsible for making sure you get these services; and
- when the services will be reviewed, to make sure your needs continue to be met.
You will be given a written copy of your Care Plan in a folder which will also include a copy of your assessment of need.
If you have agreed with us that you want to receive direct payments, you will be asked to produce your own care plan. This will then include details of the services you will arrange with your direct payments.
If you asked your local carers association for help, and support from Cumbria County Council is required, they will discuss your needs with our workers to put together a care plan.
