Going into hospital

Our workers work with people in the community and when they are in hospital.  We can make arrangements for people while they are in hospital and prepare for their return home.  This can include care for the person you look after, if you need to go into hospital yourself.

If you or the person you look after has to go into to hospital, please let your worker know.  If you do not have a worker and feel that you will need some help before you return home, ask the ward staff to put you in touch with a hospital social worker.
