Firstly, you will receive a letter acknowledging your request to see your records.
Then someone, this might be a social worker or manager; will then contact you to discuss your request. We will discuss with you what information you would like to see. If you would like to see something specific, please let us know as this will enable us to deal with your request more quickly. We will also ask you how you would like to see your records:

Finally, we may also ask you for proof of your identity to ensure you are entitled to see the information you have asked us about.
When the records are ready, we will make arrangements for you to see them. If you wish, we can arrange a meeting where we will be able to guide you through the records, explain what they mean and answer any questions you have. If you would like us to post a copy of your records to you, we can provide a written explanation to guide you through them.
As records contain very personal information, some people may need support and even counselling. We will be able to arrange this for you, if you need it.
