How do I ask for an occupational therapy assessment?

You can ask for an occupational therapy assessment through our Customer Services staff at your local Adult Social Care office.  You can also ask for an assessment for someone you know provided you have their permission to do so.

Customer Services will ask you for some information about yourself. This is called a referral and will include your name, address, date of birth, telephone number, doctor and details of your disability and the kinds of difficulties you have. We will ask you for quite a lot of detail as this will help us to decide how to respond to your referral.

If the information you provide indicates that your needs are straight forward you will be offered alternative sources of advice and information to assist you.

If however, you indicate that your needs are significant and complex your details will be forwarded to the Occupational Therapy team for their attention.
