How to Make a Referral to Adult Social Care

Members of the public and professionals can make non-urgent referrals to Adult Social Care either by:

1. Completing our online referral form, or by

2. Downloading and completing our referral form (PDF, 80Kb) and sending it to your local Adult Social Care office.

Please note that, in most cases, you must seek the consent of the person you are referring before doing so.

Urgent referrals should be made via the Emergency Duty Team.
