An exciting opportunity has arisen for a fixed term Reablement and review officer.to join our team. This is an opportunity to directly enhance and improve the well-being of individuals in local communities.
About Us
The Reablement Service provide short term support for adults with wide ranging needs enabling them to live as independently as possible whilst also ensuring their safety and well-being. We are committed to working with them to achieve person centred outcomes helping them to remain in their own homes within their local communities.
Working with us you will meet a diverse range of people and you will build strong working relationships within health services social care professionals and the Integrated Care Communities across North Cumbria.
The Role
You will be the first point of contact for those who wish to access to the Reablement Service; visiting people to carry out an initial assessment with them possibly in their home environment or in a hospital setting. You will work with them taking a holistic approach to support planning utilising a person centred and strengths-based approach.
You will discuss and identify the outcomes they would like to achieve during their reablement episode and through regular assessment and review you will monitor progress and make sure the outcomes remain achievable. Within your role you will work closely with the support worker teams occupational therapists and partner agencies to ensure the best possible outcome for the individual.
Each day and case will vary; you may support individuals to regain skills that they were able to carry out prior to referral you may support people to regain their confidence or your work may even involve supporting an adult who needs assistance to access facilities within their local community.
Who we are looking for
We are looking for people who have experience of working in a person-centred manner and can deliver exceptional service. The ideal candidates will have a good knowledge of the Reablement service and will be able to treat each person as an individual working with them to ensure their safety and dignity are always maintained.
Experience in a health or care setting is not a requirement but we are looking for people who have knowledge of assessments reviews and identifying outcomes for those who need support. This role requires a holistic approach and the right candidates will work with empathy and consideration for our Service Users.

The environment we create at Lapstone is to encourage our residents to stay active in body and mind which supports the person s independence and dignity at all times. We deliver the highest quality of care to our residents.
Working within our Residential Care Service will typically involve working shifts which usually cover between 7am until 10pm (days) or 9.30pm - 7.30am (nights) over 7 days 365 day a year - this will include alternate weekends (as part of a set rota) and Bank Holidays. 12 hour shifts can also be accommodated if this is a preference.

About the role

Blackwell Road Residential Home and is an outreach and respite service available to young people who live in Cumbria. We also offer emergency placements to young people that have nowhere else to go.
The aim of the service is to provide a planned intervention programme for young people and their families in order to reduce the risk of young people going into care and also to help prevent those young people already in care from experiencing placement breakdown.
The Role
As an Assistant Manager you will be directly involved with working towards improving outcomes for children and young people who are identified as being on the edge of care or at risk of a placement breakdown. You will oversee a team of outreach and respite support workers and supervisors and work in collaboration with a wide range of professionals to ensure the best support and the most effective communication for the children and young people.
This will include organising and processing district referrals identifying specific areas of support allocation of key workers and a review of service for each case managed. As well as managing caseloads you will chair meetings between all relevant agencies as part of the service review programme.
The work is challenging but rewarding and as Assistant Manager you will be required to lead and support your team on residential duties including sleep in duties and to promote the emotional and physical health and wellbeing of young people on respite stays.
Who we are looking for
The ideal candidate for this role will have at least three years' experience working in a residential setting for children and young people supporting those with complex needs and challenging behaviours. We are also looking for someone who has experience in a senior/managerial role who can demonstrate motivational leadership skills. The right person will need to have the ability to evaluate adapt resolve conflict and to promote equality diversity and children's rights.
An NVQ Level 4 or equivalent diploma 5 in children and young people care management is essential as is a full driving license.
